Scheduling

To request an appointment, click here. From there:

  1. Request an appointment
  2. Laurel will receive the request and confirm or decline.
  3. You will receive a confirmation email.
  4. You will receive another email with the patient intake forms.

Using online portal, complete the patient intake form PRIOR TO YOUR APPOINTMENT. Be sure to add medications with doses and allergies to medications.

Please arrive 5 minutes early for your appointment.

Scheduling for surgical appointments must be made via phone or in-person.

Medical dermatology appointments are either 15 minute or 30 minute visits. All new patients will be scheduled for an initial 30 minute appointment.

Surgical appointments are based on 30 minute increments depending on surgical complexity. Please refer to the medical dermatology and surgical dermatology pages on our website for specific pricing of services. Surgical appointments require a $50 deposit to hold the time. If the appointment is no-showed, the card will be billed a $50 non-refundable fee.

Please select the Full Body Skin Exam appointment if you have multiple concerns (more than 2) that you would like to addressed during your visit.

If your concern is not listed or if you are unclear which appointment to select, please call at 208-948-1754 to schedule.

Private Insurance/Self Pay/No Insurance

Twin Falls Dermatology & Aesthetics does not participate in, and is considered out of network with private health insurances.

Patients pay for the care they receive at the time of service. Fee schedules for medical and surgical services are posted on the website under the “Pricing” section.

Please see the links provided for the major Idaho health insurance company claim forms. We can provide you with a “super bill” and receipt, along with your claim form so that you can process the claim directly with your insurance. Your private insurance may reimburse you directly or apply the claim toward your out of network deductible.

We cannot guarantee that your insurance company will reimburse you and all questions regarding your insurance coverage and reimbursement should be directed toward your insurance company or benefits manager.

Payments

Payment in full is due at checkout for all medical, cosmetic and surgical services.

Cash and credit cards are accepted forms of payment. A credit card on file is required to book some appointments, but is not charged unless there is a violation of our cancellation policy. For cosmetic or surgical appointments, a deposit will be collected to reserve your appointment.

HSA/FSA. Health spending (HSA) and flex spending (FSA) accounts are accepted for all medically necessary services.

Cosmetic/Elective Procedures

Payment is due at the time of service. *Filler Consultation fee: $50, applied to service.

There will be a $50 fee to hold Filler appointments, which will be applied to the service. If the appointment is no-showed, the card will be charged $50.

Lab and Pathology Fees

We have negotiated discounted fees for pathology of $75 per specimen for routine skin lesions, or you may choose to use your private health insurance to submit a claim for each specimen. If the biopsy requires special stains or immune-assays, the cost will be higher. This will be billed directly by the Pathologist.

Lab/blood draw fees will be billed directly to the patient. These will be billed directly to the patient or patient’s insurance.

It is your responsibility to know which procedures your insurance will and will not cover at these facilities and to request an Explanation of Benefits (EOB) from your insurance carrier.

No-show and Cancellation Fee

We require a 24- hour notice for cancellation. Kindly follow the instructions for cancellations for our online scheduling system or call us to cancel/reschedule as necessary.

Missed visits without 24-hour notice will result in a charge of $25 fee.

Exceptions to the cancellation policy are made for emergencies.

Health Savings Account

How High Deductible Health Plans and Health Savings Accounts can reduce your costs:

If you enroll in an HDHP, you may pay a lower monthly premium but have a higher deductible (meaning you pay for more of your health care items and services before the insurance plan pays).

If you combine your HDHP with an HSA, you can pay that deductible, plus other qualified medical expenses, using money you set aside in your tax-free HSA.

So if you have an HDHP and don’t need many health care items and services, you may benefit from a lower monthly premium. If you need more care, you’ll save by using the tax-free money in your HSA to pay for it.

Your HSA balance rolls over year to year, so you can build up reserves to pay for health care items and services you need later.

Laboratory Fees

You can include in medical expenses the amounts you pay for laboratory fees that are part of medical care.

Physical Examination

You can include in medical expenses the amount you pay for an annual physical examination and diagnostic tests by a physician. You don’t have to be ill at the time of the examination.

Operation

You can include in medical expenses amounts you pay for legal operations that aren’t for cosmetic surgery.

Patient can print and bring to appointment or copies will be available.
